Before your player can participate in the HVYF&CA Fall Program, your team parent must have both the player consent form (signed by both parent/guardian and player) and physical waiver (completed and signed/stamped by physician), listed below.  If your team parent is missing either of these forms, they will notify your coach and your player will not be permitted on the practice field until they are completed and signed.

Your team parent will also need to collect your participant's final report card from their last year of school and if new to the program will need a copy of their birth certificate.
